چکیده
Retinoblastoma remains the most common pediatric intraocular tumor. While current therapies have significantly limited patient mortality, the focus has recently shifted towards eye-preserving treatments. These new eye-preserving therapies are particularly relevant to 40% of the RB cases that are bilateral. Periocular chemotherapy, selective ophthalmic artery infusion chemotherapy (SOAIC), and intravitreal (IVit) chemotherapy are relatively recent therapeutic options that have shown potential for eye-preservation. The ultimate goal of these therapies is to deliver a locally toxic dose of chemotherapeutic agent while avoiding the systemic side effects seen in intravenous chemotherapy. Periocular carboplatin has been used to deliver a high, stable con centration of chemotherapeutic agents by sub-Tenon injection. Reports indicated a favorable clinical response with tumor regression, but long-term fol low-ups have shown tumor recurrence and disease progression in the eyes treated with periocular carboABSTRACT

Combined intravitreal melphalan and topotecan for refractory or recurrent vitreous seeding from retinoblastoma.
IMPORTANCE Demonstrating the usefulness and complications of multiagent intravitreal chemotherapy is necessary for successful treatment in patients with recalcitrant vitreous seeding of retinoblastoma. OBJECTIVE To determine the efficacy and complications of combined intravitreal chemotherapy (melphalan hydrochloride and topotecan hydrochloride) for viable vitreous seeding from retinoblastoma...
متن کاملIntravitreal Chemotherapy for Vitreous Seeding Caused by Retinoblastoma
Vitreous seeding is one of the primary reasons for treatment failure following conservative management of retinoblastoma.1,2 In our hands, until early 2006, most children with recurrent vitreous seeding were managed with plaque radiotherapy, external beam radiotherapy, or enucleation.1,2 In 2006, following the initial experience of Kaneko and Suzuki in Japan,3 we performed the first intravitrea...
متن کاملEnucleated eyes after failed intra-arterial infusion of chemotherapy for unilateral retinoblastoma: histopathologic evaluation of vitreous seeding
Selective intra-arterial chemotherapy (IAC) has been adopted by many ocular oncology centers to treat advanced intraocular retinoblastoma. In this report, we describe two patients with unilateral intraocular retinoblastoma and persistent vitreous seeding, who were treated with IAC after failed systemic chemotherapy. Despite multiple sessions and increasing dosage of drug delivery, vitreous seed...
متن کاملDiffuse chorioretinal atrophy after a single standard low- dose intravitreal melphalan injection in a child with retinoblastoma: a case report
BACKGROUND Controlling retinoblastoma with seeding is challenging despite advances in treatment modalities. Intravitreal melphalan is an alternative to external beam radiation or enucleation for recurrent or refractory vitreous seeds. Significant ocular side effects following intravitreal melphalan injections are uncommon. Complications have been reported in eyes receiving higher concentrations...
متن کاملIntra-arterial chemotherapy for retinoblastoma in eyes with vitreous and/or subretinal seeding: 2-year results.
BACKGROUND/AIMS To review the effectiveness of intra-arterial chemotherapy for advanced intra-ocular retinoblastoma with vitreous and/or subretinal seeds in naive (untreated) and previously treated eyes. METHODS Retrospective study, approved by the institutional review board, of 76 eyes of 67 patients with retinoblastoma with subretinal and/or vitreous seeding treated with intra-arterial chem...
